To request your Pre-employment or Re-employment medical exam, please complete the form provided. Pre-employment medical exams are for new employees, while Re-employment medical exams are for returning employees. Once you submit your request, Workplace
Medical Corp will confirm your appointment within two business days via email. Please note that while we will do our best to accommodate your preferred times, availability is not guaranteed.
On the day of your appointment, it is essential to
bring government-issued photo identification. A passport is the preferred form of ID, but if unavailable, a valid driver’s license will be accepted. Additionally, please bring proof of your immunizations, including Influenza (if previously received),
MMR (mandatory for all crew members), and Varicella (required for Youth Activities and Nursery Counsellors). An immunization card or titre will also be required.
You must also bring your completed Disney Cruise Line Pre-employment Medical Examination
(PEME) or Re-employment Medical Examination (REME) Information form. Ensure that all applicable sections are filled out prior to your appointment. If you are currently taking any medications, please bring a list of those as well.
If you need
to cancel or reschedule your appointment, you must provide at least 48 business hours’ notice by calling 800-263-9340 or emailing operations@workplacemedical.com.
Please note that failure to show up for your scheduled appointment may result in you being charged for the cost of the appointment.
